US Passport Application Appointment Requirements

All Documents must be the original and you must bring additional copies.

  1. ALL applicants, including children, MUST be present in our office when applying for a US Passport.
  2. Form DS-11 Passport Application.Available online at travel.state.gov or in our office. Complete in black ink only. If you are printing completed Form DS-11, please print single sided.
  3. U.S. Citizenship Document Original/Certified U.S. Birth Certificate, previous undamaged fully-valid U.S. Passport Book or Card (can be expired), Certificate of Birth Abroad OR original Certificate of Naturalization. Photocopies are not accepted as original physical evidence. The U.S. Department of State requires long-form birth certificate with parental information. Please see travel.state.gov for further information on U.S. Citizenship Documents.
  4. Photo Identification Florida driver's license or review list for other acceptable primary photo IDs . A secondary form of photo identification will be required for non-Florida Driver's Licenses.
  5. One 2 x 2 Passport Photo. FGCU has a Passport Photo Booth located in the Library. The cost is $12.99 per ID Print, payable by credit card. There are also passport photo services at local establishments outside of FGCU. Please review the U.S. Passport Photo requirement for further details.
  6. YOU MUST provide COPIES: Copies (single sided) - Copy of the front & back of photo ID on 81/2 x 11 sheet of paper and copy of the front & back of U.S. Citizenship document on an 81/2 x 11 sheet of paper. DO NOT ENLARGE copies

TWO SEPARATE PAYMENTS are Required:

  1. Check or money order payable to the U.S. Department of State. NO CASH PAYMENTS, starter checks, or credit/debit card payments. This payment gets mailed with the passport application. Please see the Passport Fee Chart 2023for further details.
  2. Check, money order, or credit/debit card payable to FGCU. $35 FGCU Facility Execution Fee. You have an option to add USPS Express Mail (1-2 day delivery) for an additional $26.35 = $61.35 FGCU Fee with Express Mail.
    • Money orders can be purchased at many local establishments such as banks, local supermarkets, and post offices.

Applicant's Age 16 or 17 eligible for Adult Passport:

  • Parental Awareness: The applicant and at least one parent/guardian along with their photo identification must be present at the time of the Passport appointment. Both parents along with their photo identification are helpful in the passport processing of children age 16-17. Please let us know if only one can attend for further instruction.
  • Copies (single sided) -Copy of the front & back of both parent(s)/guardian(s) photo ID on an 81/2 x 11 sheet of paper. DO NOT ENLARGE

Applicant's Under Age 16:

  • Child applicant and both parent(s)/legal guardian(s) must be present at time of the appointment to sign off on the application.
  • Both Parent(s)/Guardian(s) Photo Identification and photocopy of front and back of ID printed single sided on an 81/2 x 11 sheet of paper. DO NOT ENLARGE copies
  • Child's original/certified U.S. birth certificate and photocopy of front and back single-sided on an 81/2 x 11 sheet of paper.

ADDITIONAL REQUIREMENTS:

Proof of Name Change: If your name has changed for any reason (marriage, adoption, etc) you will need to bring in original documentation to support your name change (marriage certificate, adoption papers, court order name change documents, etc).

  • Enter through the main FGCU campus entrance via Ben Hill Griffin Parkway.
  • A Parking Permit is required to park on campus. Please stop by the FGCU Information Booth at the FGCU main entrance for your parking permit.
  • Turn right at the stop sign onto FGCU Blvd. South, after you get your parking pass.
  • Our office is located in Modular 1 building on your left after you pass the Campus Support Complex blg.

2. Documento original de prueba de cuidadanía(en inglés)

  • Ejemplos -Certificado de nacimiento de EE. UU., pasaporte estadounidense vencido, certificado de naturalización.

3. Identificación con foto

  • Ejemplos -Licencia de conducir de la Florida, pasaporte estadounidense (válido o vencido) o pasaporte extranjero (válido), certificado de naturalización o de cuidadanía, identificación de gobierno, matricula consular o tarjeta de residente permanente de EE. UU.

4. Fotografía para el pasaporte

  • Una foto reciente de 2" por 2" con fondo blanco. Las fotos de pasaporte se pueden tomar en los centros de fotographía de Walgreens y CVS.

5. Pago de la tarifa por el trámite del pasaporte

  • Cheque o giro postal pagadero a U.S. DEPARTMENT OF STATE

    • Servicio Urgente - $60.00 adicional

6. Copias del documento de prueba de cuidadanía y identificación.

OUR OFFICE ONLY ACCEPTS APPLICATIONS FOR NEW PASSPORTS

ADULT RENEWALS MUST BE MAILED BY THE APPLICANT THROUGH THE U.S POST OFFICE. Renew your Adult U.S. Passport by mail with the DS-82 Form.

*If your previous passport was issued when you were under the age of 16 you must apply for a NEW passport in person.

  • How do I check the status of my passport application?

    The FGCU Passport Application Facility Office does NOT have access to passport information after the application has been mailed from our office. For Passport Status, please visit www.travel.state.gov or contact National Passport Information Center at 1-877-487-2778 or NPIC@state.gov.

    • Enroll in email updatesso you know when your application status changes. Updates may not be available in the first 2 weeks after you submit your application. During these 2 weeks before your application arrives at a passport agency or center, your application goes to a mailing facility and an intake facility where your payment is processed.

  • How long will it take to get my passport?

    Processing times vary throughout the year depending on demand. At peek times, it can take up to 4 months for your passport application to be processed so it is important to start planning early.

  • What is the difference between a Passport Book and a Passport Card?

    The U.S. passport card is a wallet-sized, plastic passport that has no visa pages. The card is proof of U.S. citizenship and identity, and has the same length of validity as the passport book.

    The card is for U.S. citizens who travel by land and sea from Canada, Mexico, Bermuda, and Caribbean countries.

    • The card isnot valid for international travel by airand is cheaper than the passport book.
    • The passport card is only valid for land border crossings and sea ports-of-entry in Canada, Mexico, the Caribbean, and Bermuda.
    • A passport book is valid for all international travel by air, sea, or land.
    • You can apply for either the book or card, or both documents.

    Our office cannot process anyone's application that requires immediate travel (two weeks or less) or if the travel dates are less than the processing times needed.

    Urgent requests can only be made through the U.S State Department

    The U.S Department of State has an Online Passport Appointment System. The closest Pasport Agency is in Miami and there is a VERY limited number of appointments. The process to make an appointment depends on several factors:

    • You may need to schedule an appointment with them if you are traveling within 14 days or need a foreign visa within 4 weeks
    • You will need to provide proof of travel to them(such as an airline ticket receipt or itinerary) at the time of your appointment. If you are traveling by land or sea to Canada, Mexico, Bermuda, or the Caribbean, you can provide a hotel reservation, cruise tickets, or international car insurance.
    • First, try tomake an appointment online with the Miam office if you have not applied for a passport.
    • If you are unable to make an appointment online or you have already applied for a passport, call1-877-487-2778on Mondays through Fridays from 8:00 a.m. to 8:00 p.m. (ET). If you are deaf or hard of hearing, call1-888-874-7793for TDD/TTY teletype services.
    • Call202-647-4000if you have a life-or-death emergency after 8:00 p.m. (ET) on Mondays through Fridays, or on Saturdays, Sundays, and federal holidays. Do not call this number between 8:00 a.m. and 8:00 p.m. (ET) on Mondays through Fridays, or if you needUrgent Travel Service.

  • What if I lost my passport?

    Report Passport Lost or Stolen

    If your passport is lost or stolen you must report it to the State Department right away. How you replace it depends on if you are inside or outside the U.S.

    Report your valid U.S. passport lost or stolen immediately. Protect yourself from identity theft.

    • Use one of three ways to report it lost or stolen: Online, By mail, orIn person when you apply for a new passport.
    • Do not report a passport lost or stolen if it has expired.
    • When you report your valid passport lost or stolen, the US Department of State will immediately cancel it. You cannot use it for international travel even if you find it later.

    Want to immediately replace your valid lost or stolen passport?Reporting it lost or stolen does not replace it. You mustapply in person, pay the fee for a new passport and submit both Form DS-64 and Form DS-11 if you want a replacement passport.
